Modivcare provides free non-emergency medical transportation for NJ FamilyCare clients across New Jersey.

How to Book a Ride:

• Call 1-866-527-9933 at least two days before your appointment.

• Book online at tripcare. Click here!

Have the following information ready:

• NJ FamilyCare ID number

• Pick-up address and zip code

• Medical provider’s name, phone number, and address

• Appointment time and date

• Any special transportation needs

Who Can Apply:

• Seniors 62 years or older (65 for Metro-North Railroad stations)

• People with disabilities

• Personal assistants (ride for free with proper ID)

How to Apply:

• • Show a valid NJ TRANSIT Reduced Fare ID or Medicare Card to the operator.
