What to Bring to a WIC Appointment?
When you visit, please ensure that all applicants in the family, including children, are present for the appointment. You will also need to bring the following materials:
Proof of identity
Proof of pregnancy
Proof of income
Proof of residency
Your health care referral form (if completed by your doctor or other health care provider)
Your children's medical and vaccine records
Services: Healthy pregnancy and new parenthood support, free doula services, donations of diapers, childhood lead poisoning prevention.
